- Philippine Early Childhood Development Checklist (PECD) for incoming GRADE 1
- Copy of latest Report Card and with Learner’s Reference Number (LRN) and with signature of Adviser/School Head.
- Certificate of Good Moral Character issued by the principal or school head.
